Phase Conversion of Service

If an existing MVEA service needs to change for any of the following reasons, a Phase Conversion of Service Request is needed:

Three phase service to single phase service
Single phase service to three phase service
Changing a three phase 120/208 voltage service to 277/480 voltage service or vice versa

Getting Started

Download and complete a Load Data Sheet (PDF) which can be found below for your new load.
Complete and submit a “Phase Conversion Application Request” form found below and attach the completed Load Data Sheet to this form.
After the Engineering Department receives your request, our team will review and finalize your application.
Our team will send you an application document, which requires your signature, along with an engineering fee payment of $500.
Once our team has received your signed application and the $500 engineering fee payment, we will schedule an onsite meeting with you and one of our Line Designers to discuss your projects’ needs.

Load Data Sheet

A Load Data Sheet is required for all MVEA electric projects. Completely fill out and sign the Load Data Sheet which can be found below before submitting it to MVEA. Please remember to sign the load sheet before submitting it to MVEA.

We have three convenient ways to submit Load Data Sheets:

Information on the $500 Engineering Fee

The engineering fee will be applied to the overall cost of the project.
If the project is canceled, the engineering fee will be forfeited.
