Phase Conversion of Service-DRAFT

If an existing MVEA service needs to change for any of the following reasons, a Phase Conversion of Service Request is needed:

Three phase service to single phase service
Single phase service to three phase service
Changing a three phase 120/208 voltage service to 277/480 voltage service or vice versa

Getting Started

Download and complete a Load Data Sheet (PDF) for your new load.
Complete and submit a “Phase Conversion Application Request” form found below and attach the completed Load Data Sheet to this form.
After the Engineering Department receives your request, our team will review and finalize your application.
Our team will send you an application document, which requires your signature, along with an engineering fee payment of $250.
Once our team has received your signed application and the $250 engineering fee payment, we will schedule an onsite meeting with you and one of our Line Designers to discuss your projects’ needs.

The engineering fee will be applied to the overall cost of the project.
If the project is cancelled, the engineering fee will be forfeited.
